Booking, scheduling and non-refundable deposit terms, by checking the box in the form, I understand:

My deposit is non-refundable.
I may reschedule up to 72 hours prior to my scheduled appointment.
I may reschedule one (1) time prior to 72 hours before my appointment.
If I cannot attend my rescheduled appointment that I will forfeit my deposit.
Cancellations and/or no-shows will result in forfeiture of the deposit in full.
I am sure of my availability prior to booking.
I should only leave a tattoo deposit after I have been asked to by my tattoo artist; or, if I have already filled out a new tattoo appointment form and have been directed to leave a deposit to secure my tattoo appointment.
